Intake Form for New Clients: New clients will receive an emailed intake form 24 hours before their first appointment. This form must be completed and submitted prior to your appointment. If the form is not completed beforehand, you will need to fill it out during your appointment time, reducing the time available for treatment. Consent, Intake & Policy Forms: These forms must be submitted before your treatment. Legally, we cannot begin your session until they are completed.
